Vue postérieure droite des nerfs sous-costal (en haut) et ilio-hypogastrique (et en bas). Ils abandonnent un rameau perforant à une distance variable de la crête iliaque. La séparation est bien visible et est marquée par les deux flèches noires supérieures. On notera que le rameau perforant de L1 est plus épais que le nerf principal (après la séparation), probablement en raison d'un territoire d'innervation plus étendu. La ligne pointillée marque la crête iliaque. Le rameau perforant de L1 la croise à sa partie la plus latérale (à la verticale du grand trochanter), celui de T12 plus en avant.
This is a right posterior view of the the subcostal (above) and iliohypogastric (below) nerves. They abandon a perforating ramus at a variable distance from the crest. The separation is clearly visible and is marked by the upper arrows. Interestingly, the perforating ramus of L1 is thicker than the primary nerve (considered after the separation), supposedly because the territory of distribution of the former is larger. The dotted line shows the iliac crest. The perforating ramus of L1 crosses it at its most lateral portion, vertically above the trochanter, the one of T12 more anteriorly.

 

Vue latérale droite des deux branches perforantes. Celle du nerf ilio-hypogastrique (L1) devient superficielle en croisant la crête iliaque à travers un orifice ostéo-aponévrotique, constitué en bas de la crête elle-même et en haut de l'aponévrose du grand oblique. Celle du nerf sous costal (T12) est déja superficielle et sous cutanée avant de croiser la crête.

Right lateral view of both perforating rami. The one of the iliohypogastric nerve (L1) becomes superfical when crossing the crest, through an osseoaponeurosis orifice constituted of the crest itself below and the aponeurosis of the oblicuus externus above. The one of of the subcostal nerve is already superficial and subcutaneous before crossing the crest.

Même spécimen après dissection de l'orifice et résection des nerfs. Une gouttière osseuse apparaît, palpable in vivo, directement au dessus du grand trochanter, sur la crête.

Same specimen after dissecting the orifice and removing the nerves. A bony groove appears, which is palpable in vivo, right above the trochanter, onto the crest.

 

Vue latérale droite. Croisement de la crête. Le nerf apparaît entre la crête en bas et l'aponévrose du muscle oblique externe en haut (flèche noire).

Right lateral view. Crossing the crest. The nerve appears between the crest below and the aponeurosis of the muscle oblicuus externus above (black arrow).

Vue antérieure gauche. L'épine iliaque antéro-supérieure est sur la gauche. Le muscle aux fibres verticales est le moyen fessier. Le nerf était comprimé par l'arche aponévrotique (voir page suivante, photo du bas).

Left anterior view. The antero superior iliac spine is on the left. The muscle with vertical fibers is the gluteus medius. Tne nerve was pinched by the aponeurotic arch (see next page, bottom view).
